A chart entry is the record of one visit: the treatment note, photos, products used and files. It saves as you type, and it locks once it’s signed or marked as complete.

Start an entry

A service that’s checked under Charts by service in Settings, Charting starts an entry when it’s booked, already on its template. To start one yourself:
1

Open Charts

In the menu, go to Charts.
2

Select Create chart

Select Create chart, choose the Client, then select Create chart.
You can also start one from a client’s profile: open the Chart tab and select Add beside Charts.

Write the entry

1

Jump to a section

Use the row of section names at the top (Chart details, Photos and markup, Treatment note and the rest) to jump to a part of the entry.
2

Fill in the treatment note

Under Treatment note, choose a template in Select template if the one shown isn’t right, then write under each heading.
3

Add photos, products and files

Add photos under Photos and markup, what you used under Product details, and lab results or referrals under Files. To map injection sites on an outline, select More, then a diagram such as Diagram: Face, front. On a tablet or computer it opens ready to mark up. Select Before or After under a photo to label it in one tap.
The line under the client’s name says where the entry stands and what’s next. The header shows Saving… and then Saved as you type. There’s no Save button.

What each status means

  • Not started: nobody has written in the entry yet.
  • In progress: it has been written in and isn’t signed yet.
  • Ready for review: the provider signed it and it’s waiting for the Medical Director.
  • Completed: it’s signed or marked as complete, and locked.
  • Amending: someone is changing a locked entry. See Sign a chart entry.

Good to know

  • A locked entry can’t be edited. To correct it or add to it, select Amend chart. The original stays in Version history.
  • An entry that’s Not started or In progress can be deleted from the list’s row menu, with a reason. A locked entry can’t be deleted.
